Fall Sports Information

Please read the following information if you are interested in participating in fall sports at Holmes for the 2023-24 school year:

Mr. Olschanski (Athletic Director) will be sending out an email for Fall Sports sign up on August 21, the last day to sign up will be August 25.

All students must have a signed, valid sport physical form on file in the Athletic Office in order to participate.

To be valid for the 2023-24 school year, a  sport physical must be dated AFTER APRIL 15, 2023.

Download LPS Physical form (Please print both pages and read the entire form.)
Be sure to take this form to the doctor to fill out medical information and sign and date.
The form WILL NOT be valid without a doctors signature and appropriate date.
